School Health Forms

Medication Authorization Form

The Medication Authorization Form must be completed for any student who is required to take medication at school. This form must be completed by the student's physician and requires a physician's office stamp. A complete form is required for any medication, including over-the-counter-medications, excluding Tylenol and Motrin. All medication orders must be renewed each school year, and the parent/guardian is responsible for bringing in and picking up medication.

As mentioned, a single dose of Tylenol or Motrin can be administered at school without a Medication Authorization Form. However, written parent/guardian permission is required. Please provide written permission via Genesis.

Asthma Treatment Plan

If a student has asthma, please ask their physician to complete the Asthma Treatment Plan in addition to the Medication Authorization Form.

Allergy and Anaphylaxis Emergency Plan

If a student requires an Epipen for a life-threatening allergy, please ask their physician to complete the Allergy and Anaphylaxis Emergency Plan in addition to the Medication Authorization Form.

Universal Child Health Record

A complete Universal Child Health Record is required for every new student. Most pediatricians will have this form at their office.

Sunscreen Authorization Form

The Sunscreen Authorization Form must be completed for any student to be permitted to self-apply sunscreen at school or for any child who may need assistance or supervision with this task. 

Preparticipation Physical Evaluation Form (updated) - For Students Entering Grade 6

Preparticipation Medical Eligibility Form (updated) - For Students Entering Grade 6

If your student is entering Grade 6 next year and would like to participate in athletics, please ask their physician to complete the Preparticipation Physical Evaluation Form and Medical Eligibility Form. The Physical Evaluation Form should be kept at the physicians office and the Eligibility Form should be returned to the school nurse's office. These forms are new in 2024. Old forms will be not be accepted after 06/30/2024.
